About Us > First Time Visitor?

As a new patient we need to understand your medical history and the care you have recently received so we can provide you with the most appropriate and effective care. Please bring the following with you.

Medications

Please bring either a complete list of all you medications including any herbal and vitamin mixtures or bring the bottles with you.

Insurance card

Please bring your insurance card(s). If your insurance company requires a referral for your visit, please make sure your referring doctor has provided this.

We will file your insurance claims on your behalf, and help you with questions or concerns regarding payment or your bill.

Medical records

Please arrange to have your previous test results and reports sent to us. Some doctors will directly send them to us on your behalf; others will give records to you to bring with you to your first visit. An example would be any or all of the following: x-rays, CT scans, PET scan, laboratory work, or pathology report.

Questions and concerns

Please bring some one with you to your first visit because it always helps to have a second person listening. A list of written questions to review with the physician may also be beneficial. You can bring a tape recorder if you desire but please inform the physician that you would like to use one. Our physician will give you whatever time is necessary to answer all your questions and concerns regarding you condition and treatment plan.
